Mrs. Ana R. came into Cortlandt Healthcare on a snowy December day. Admitted from Hudson Valley Hospital Center, we learned that Ana R. was in need for rehabilitation for her sciatica and difficulty walking. Ana R. came to the community with minimal ability to walk on her own, but her spirits were high and she was ready to get better again!

With therapy by her side, and family to support her, Ana R. made goals that she not only set, but achieved with pride so she could be on her journey home. Ana R. needed to be able to go up and down stairs, walk a certain distance, and be able to transfer safely and independently. The baseline for Ana R. upon admission was a maximum assistance of one to help ambulate. Just a little over two weeks, Ana R. was able to perform a safe transfer on her own.

The next goal that had to be met was walking over a certain distance. Ana R. had a baseline of being able to walk 40 feet with a rolling walker. In just two short weeks, and a lot of motivation, Ana R. was able to ambulate 200 feet with her rolling walker! After Ana R. was steady on her feet and gained her strength back in her legs, she was able to give the stairs a try. After a few attempts, and a week later, Ana R. was able to ascend and descend eight steps with both hands on the hand rails.

Throughout Ana R.’s experience she shared that, “there are such nice people around. Everybody is so friendly and happy! My physical therapy was very nice; excellent actually! I feel so happy!”

Case Study: Cortlandt Healthcare (December 2017)

Details of Experience:

Upon arrival, Joseph S. was admitted into Cortlandt Healthcare with a femoral fracture to his left leg, he had a history of falling, muscle weakness, and difficulty walking. Joseph S. had baseline strength of only ambulating 150 feet with a contact assist and had an antalgic gait pattern. Over the course of the next two and a half months he was able to improve his gait and ambulate up to 250 feet with a stand by assist! Throughout Joseph’s stay, therapy noted that he demonstrated excellent rehabilitation potential and had a strong family base and support, which he could use as motivation to participate in his therapy.

Once those goals were met and completed, it was time to move on to the car transfer to ensure the best and safest discharge home for Joseph. The initial evaluation, Joseph was able get in the car with assistance. Two weeks later, Joseph was able get in and out and able to complete a safe car transfer on his own! Throughout his therapy, Joseph facilitated independence in all of his self tasks such as getting dressed, bathing techniques, dressing techniques, and overall hygiene and grooming techniques. Joseph resided in Cortlandt’s grand King Suit and shared, “the room is absolutely beautiful!”

Joseph also spoke about his experience and shared, “everyone in therapy is so kind, considerate, and professional. I am very impressed with everyone! Everyone has been exceptional! Overall, I would have to say my experience was excellent and very satisfying!”

Celebrating Longevity and Beauty!

On Friday, December 22nd, Mary M. celebrated her 105th birthday alongside her loving family and neighboring residents at Cortlandt Healthcare. Mary was born in 1912. It was a year of many significant events including that of Woodrow Wilson being elected as president, Arizona becoming the 48th state in the U.S., the Republic of China being founded, and the Titanic making its fateful voyage. Mary has lived through many incredible historical events and has even secured newspaper clippings from some of the most memorable. It seems an amazing feat, but Mary knows a great deal about longevity; her parents and older brother both lived to be over 100 years old!

Mary was born and raised in Ossining, New York. In fact, she was an Ossining resident living without assistance in her own home until the age of 99! When her husband, Harvey, passed away, she moved to Cortlandt Healthcare to remain active and social. It is at Cortlandt Healthcare that she enjoys sitting in the hallway greeting residents and visitors. She looks forward to attending the daily events and especially enjoys activities with music.

In her younger years, Mary was a homemaker with a love of cooking. She would often be found in her kitchen making her favorite Italian dishes and desserts. She enjoyed reading; Reader’s Digest was one of her favorites. She never missed 60 minutes and couldn’t pass up an episode of “Golden Girls.”

It is Mary’s family that is her greatest accomplishment. Mary is the mother of one daughter, Marlene, Grandmother of two, Gregory and Jillian, and great-grandmother of five: Lorenzo, Katya, Angelo, Myles and Brooks. Mary is not a big fan of the question, “What is the secret to your longevity?” because she just doesn’t have an answer. If you ask her daughter she does have an inkling…Mary never wants to miss a family milestone and wills herself to wake up each day to hear about her great-grandchildren and their latest achievements. We wish you many more years of happiness and beauty in life, Mary. Happy Birthday from your Cortlandt family!

Case Study: Cortlandt Healthcare (November 2017)

How did this patient hear about Cortlandt Healthcare? Thomas T. knew about Cortlandt Healthcare as he was a patient previously in 2013.

Details of Experience:

During Thomas T.’s stay at Cortlandt Healthcare, he was very determined to regain his strength and eventually be discharged home. Thomas T. came to therapy with a right and left knee brace and chest pain. His baseline in therapy was fair and required a one person assist, but had some difficulty shifting his weight. In the beginning, Thomas T. was only able to walk a total of five feet and was not able to use the stairs.

After receiving physical therapy six days a week for a few weeks, Thomas T. was able to gain momentum and muscle in his legs. He progressed from walking five feet to 100 feet just four weeks later, and he was also now able to walk 150 feet by November 20th!

Once Thomas T. was able to get up and walk, the next goal were the stairs. Thomas T. had to set a goal to overcome multiple stairs in order to qualify for a safe and approved discharge home. During his eighth week, Thomas T. was able to walk up and down a total of 16 stairs! Lastly, Thomas T. completed his goal of a car transfer safely and independently the same week!

Outside of therapy, Thomas T. enjoyed playing bingo and meals in the day room with the other patients. During his initial, weekly, and monthly concierge evaluations, Thomas T. described the nursing staff as, “excellent, they are all very good and they always do what I ask!” Thomas T. describes his physically therapy experience as, “they are excellent and always on top of my exercises. They are fantastic and such professionals!” Overall, Thomas T. rated his experience a 10 out of 10 for satisfaction!

Details of Experience:

L. Sanabria arrived to Cortlandt Healthcare at the age of 72 seeking physical therapy for his recent diagnosis of bronchopneumonia and muscle weakness. During treatment, L. Sanabria required physical therapy six times a week for at least four weeks. The initial goals that were anticipated were to safely ambulate 75 feet using a rolling walker, then increase steps to 150 feet, and with assistance, go up and down seven steps. Over the next couple of weeks, therapy worked hand and hand with L. Sanabria to increase his gait and overall balance and strength. On August 1st L. Sanabria was able to ambulate 250 feet with a rolling walker! One month later, on September 1st, L. Sanabria was reevaluated and able to ambulate 400 feet with a rolling walker! He not only met his goal, but exceeded it and his expectations.

The next goal L. Sanabria had to meet and overcome were the stairs. A goal was set of seven steps; by August 28th he was able to achieve the seven steps while holding the sides of the handrails. When speaking to L. Sanabria he explains his time here was “a big experience.” L. Sanabria continued to share, “physical therapy went very well. My therapist, Bob; he is my buddy. He did beautifully and he helped me out so much. Nursing has been very good and they always helped me out.” Overall, L. Sanabria shared that “everything went perfect and I am very happy!” We wish him nothing but the best as he continues to heal and strive for greatness!

Details of Experience:

D. Holloman came to Cortlandt admitted from Hudson Valley Hospital Center with a diagnosis of an abscess liver, muscle weakness, paraplegia, and essential hypertension, which caused her to have a PICC line in place and a JP drain. At the age of 83-years-old, D. Holloman was living alone and needed help gaining her strength back during her recovery. During her stay, D. Holloman received therapy six times a week and made great progress.

Upon admission, D. Holloman had difficulty walking and arrived walking a total of 60 feet. At the time of graduation and her discharge home, she was up to 350 feet! Throughout the weeks D. Holloman is able to maintain good balance while standing up without any support and eventually continued on and graduated at the highest practical level for her therapy.

D. Holloman’s physical and occupational therapy worked hand-in-hand to ensure she was aware of all the safety precautions in order to enhance the highest quality of life for her. They also worked on improving the ability and knowledge to safely return home to her private residence.

D. Holloman successful discharged home, but before doing so she expressed her experience of her journey to us. D. Holloman shared that during this time it was a learning experience and the rehabilitation was excellent and the staff were wonderful. She also shared her experience by saying, “my whole experience was overall very good. I learned a lot and physical therapy was excellent. I got along with everyone and everyone (staff and patients) treated me like a mother and a friend, it was so nice.”

End-Of-The-Summer BBQ a Great Success!

The Cortlandt Healthcare community was excited to host an end-of-the-Summer Labor Day BBQ for staff, residents and families on September 5th!

As another Summer season came to an end, Cortlandt celebrated the Labor Day holiday with delicious barbecue and festive music. Those in attendance enjoyed a scrumptious spread of fresh hamburgers, “all-American” hotdogs and barbecue chicken, a crowd favorite, wonderful potato salad and coleslaw, succulent watermelon and sugar-free cupcakes!

Cortlandt’s Annual Summer Luau BBQ!

Cortlandt Healthcare was excited to host their festive Summer Luau Barbecue in celebration of the exciting season at the community! Residents, staff and families were treated to delicious sweet & sour pork and chicken teriyaki with fresh garden and fruit salads! A scrumptious pineapple upside down cake was also served!

Entertainment provided by the fabulous Sue Larsen made us all feel as if we were enjoying the day in Hawaii!’

Talented Cellist Visits Cortlandt Healthcare!

Howard Cohen, talented cellist within the community, paid a special visit to Cortlandt Healthcare this week! Cohen was kind enough to perform for our residents and staff while also providing a quick lesson. We had a blast! Check out his performance!
