Details of Experience:

Ernest was admitted to Northern Westchester Hospital after Joint Replacement Surgery. Ernest was evaluated by Northern Westchester Hospital very thoroughly and recommended physical and occupational therapy to regain his prior level of function. Ernest was recommended to Cortlandt Healthcare due to our outstanding rehabilitation program. Edmond’s main goal is to get back on his feet and go home.

Upon arrival, Ernest was greeted by the Concierge, Rehab Team, and Nursing team, who ensured he had everything he needed to start his voyage to renewal here at Cortlandt Healthcare. The Concierge assisted by making sure Ernest received everything during his stay, which included proper food/dietary options, making sure he received and was brought to recreational activities if he wished to attend, and making sure he was receiving medications on time as well as assistance when needed from the nursing team. Ernest was assessed by our Rehabilitation team and received short and long-term goals to monitor his progress during his rehab process. Physical Therapy’s short-term and long-term goals included ambulating on level surfaces, car transfers, and bed mobility tasks. Occupational Therapy’s short-term and long-term goals include functional activities of choice, toileting tasks, and practicing lower body dressings.

During Ernest’s stay, he received both Physical and Occupational Therapy. Starting with Physical Therapy, Ernest progressed to maximum assist with bed mobility tasks to minimum assist. He could ambulate on level surfaces starting at 0 feet and ending at 40 feet with minimum assist. Ernest performed car transfer tasks with supervision to minimum assist. As far as Occupational Therapy goes, Ernest went from 0 to 5 minutes of functional activities of choice. Ernest went from maximum to minimum assist with toileting tasks. Ernest went from maximum assist to moderate assist with lower body dressings while using adaptive equipment. Ernest made consistent efforts to achieve all long-term and short-term goals during his stay at Cortlandt Healthcare.

Ernest was discharged to his home on March 24, 2023, after 17 days of rehabilitation. Ernest had a wonderful experience throughout his stay, making friends with both staff and other residents. He had nothing but the best experience while residing here at Cortlandt, and he enjoyed all events and activities held at Cortlandt.
