We had a resident who was admitted from the hospital where she was admitted for a surgical repair. Upon admission to CHC this resident presented with significant loss of strength, decreased functional mobility and a lot of pain at the surgical site. This resident’s goal was to return to home at her prior level of function, living independently and active in her community.
This resident received skilled OT and PT as well as skilled nursing care. After 18 days of excellent rehab and care she was able to return home with her prior level of function!!
