Reason for Stay O2 dependance, Advanced Pulmonary Fibrosis

This resident was admitted to CHC on 4/16/15 after being hospitalized for a sudden onset of shortness of breath, possible pneumonia and a fever. This resident is oxygen dependant and also has advanced pulmonary fibrosis. Upon the initial evaluation from both physical and occupational therapy this resident was found to have decreased strength, decreased functional mobility, decreased balance, reduced participation in her activities of daily living and multiple complaints of pain. After 3 weeks of skilled physical, occupational therapy and skilled nursing care, this patient was able to reach all of her rehab goals, was stable medically and was discharged home at an independent level of mobility with the use of a rolling walker!!
