Pauline was admitted to Hudson Valley Hospital after officially being diagnosed with an Acute Embolism and Thrombosis in her left leg. Acute Embolism and Thrombosis are when a blockage (blood clot) is formed in body parts and can travel to the lung. Pauline was thoroughly evaluated by Hudson Valley Hospital and recommended physical and occupational therapy to regain her prior level of function. Due to our outstanding rehabilitation program, Pauline was recommended to Cortlandt Healthcare for recovery. Pauline’s main goal was to be able to function independently once again and to get back home as soon as possible.

Upon arrival, Pauline was greeted by the Concierge and Nursing team, who ensured she had everything she needed. The concierge helped by providing that Pauline received everything during her stay, including proper food options, that she had Recreational Activities available, and that her medications arrived on time. Pauline was assessed by our Rehabilitation team and received short and long-term goals to monitor her progression during her rehab process. Short and long-term goals include stand pivot transfers, stair assessment, bed mobility, and car transfers.

During Pauline’s stay, she received both Physical and Occupational therapy. Pauline progressed from maximum assist supervision with all stand pivot transfers. She could ascend and descend from 0 to 8 stairs and ambulated 0 feet to 50 feet under supervision. Pauline also progressed from maximum to independent on all bed mobility tasks and was able to perform car transfers from 0 to contact guard assist. Working Hard with occupational Therapy, Pauline went from maximum to minimum on all indoor home maintenance tasks. Regarding toileting tasks, Pauline went from maximum to minimum. During her stay at Cortlandt Healthcare, Pauline made consistent efforts to achieve all long-term and short-term goals.

Pauline was discharged to her home on June 20, 2022, after 46 days of rehabilitation. She had a wonderful experience throughout her stay by making friends with both staff and other residents. Pauline was able to walk out of Cortlandt Healthcare with her head held high, and she will be missed.
